Thanks, patch was applied.

If you are interested, look also at the last added commands to kamdbctl, such as grant, dbonly, etc... because I had access only to mysql to implement them. Maybe it just works for postgres or needs some trivial patch.

I tested this on Kamailio 4.0.1, but it seems that the issue exists in the latest git as well. The attached patch causes kamdbctl dbuid to successfully create the uid tables.
