Summary: RR's "append_fromtag" must be enabled for uac_replace_from

Initial Comment:
The documentation needs a big fat warning that "append_fromtag" from the rr module MUST be enabled for automatic restore for uac_replace_from to function correctly.

Additionally, it would be nice if the configuration parser would fail when the option is disabled. (Or automatically enable it when necessary)

I got random memory locations copied into the To: or From: header when it was disabled.

Tested version: 1.2.2

ok - I made the fix for both 1.2 and 1.3 ( better have better code than
docs ;) ). I made the same fix for acc module which also depends on this
parameter.

I will try to add some validation tests, but it will be only for 1.3 - for
1.2 we can add just more docs.
